  • Care Coordinator C.N.A. - Days, Shifts: 5 days/week; 8Hr days 800A-4:30P (approx), 30min lunch break (clock out) & 2 - 15min paid breaks
  • Must have experience/knowledge of medical Terminology. No call. Every other weekend.
  • Must have strong acute care hospital experience. At least 3 years of acute care hospital experience and case management!
  • Previous travel experience highly desirable.
  • Must have utilization review & discharge planning experience. Nurse will need to be able to do UR as well as oversee DC planning.
